A CAD app's REST API answered a hole cut at the wrong coordinates with 201 and the unchanged solid, which no script can tell apart from success. The server also stored geometry the kernel called invalid into the B-rep that later fillets and STEP exports read, so the failure landed on whoever opened the project next.
